Incident Narrative

A mass-exposure event in a five-story San Francisco residential complex resulted in 50 evacuations and one casualty. The incident was sustained for nearly 48 hours because an active audible alarm was dismissed by occupants as a standard alarm clock.

Technical Findings

The Carbon Monoxide (CO) buildup was traced to a mechanical failure in the basement boiler and water heating ventilation system. Due to the high concentration, toxic gas migrated vertically through the building's infrastructure, affecting multiple floors simultaneously.
